Delaware Psychiatric Center Former Employees Charged In Connection to Patient's Death

A grand jury has indicted three former workers at the state-run Delaware Psychiatric Center in connection with a patient's death.

The indictment issued this week charges Andre C. Jackson, Daniella Seneus and John Iziken with crime against a vulnerable adult, a felony, and misdemeanor offenses of forgery, falsifying business records and patient abuse or neglect. It was not immediately clear whether they have lawyers.

Prosecutors allege that the three did not check on a 38-year-old male patient as required. The man was found unresponsive in his room on December 11 and pronounced dead a short time later.

Prosecutors say the defendants failed to carry out required "face checks" on patients every 15 minutes during the previous night shift, but signed log sheets indicating that all the checks had been conducted.
