Ex-State Worker Makes Fake Insurance Cards: Prosecutors

A former New Jersey state employee has been indicted on charges that she stole more than $4,000 in assistance benefits and used her work computer to create bogus insurance cards.

Prosecutors say 38-year-old Laquanda Tate was indicted Monday on charges of official misconduct, false government documents and related offenses. Officials say that between April 2009 and December 2012, Tate used her work computer to make false insurance cards for herself and three others.

Prosecutors say she also created bogus notarized letters that she used to claim unreimbursed childcare expenses. She received $3,600 in food stamp benefits through the scheme. She also received $1,000 in housing benefits she wasn't entitled to.

Tate is a former payroll clerk for the Department of Human Services.

Her attorney couldn't immediately be reached by NJ.com.
