Officials say a foster-care worker falsified documents following the death of a 10-year-old foster child and a Philadelphia school nurse failed to report the boy's condition which could've helped him.
Ethan Okula died in February after telling a school nurse he had a stomach ache.
The Philadelphia Inquirer reports a Department of Human Services report released Tuesday says the nurse at Julia De Burgos Elementary School in Fairhill should've noted Ethan had abdominal scarring, which was causing bowel obstruction and lead to his death.
The report says the nurse never filled out a form detailing his condition and care instructions.
The report says a Northeast Treatment Services worker later described a meeting with caseworkers that should've happened and never did. It would've outlined goals for Ethan's care and served as a check-up on his well-being.
