NJ Companies Fined After Man Dies in Chocolate Vat

Camden chocolate company had no first-aid program in place, says OSHA

Two Camden chocolate companies have been fined almost $40,000 for safety violations related to the death of a Camden man who fell into a vat of melted chocolate last July.

The U.S. Department of Labor's Occupational Safety and Health Administration cited Lyons & Sons Inc. and Cocoa Services LP with a total of 12 serious citations. They were fined more than $39,000.

Vincent Smith II, 29, died July 8 when he was standing on a nine-foot platform loading chunks of chocolate into a melting tank at Cocoa Services in Camden, N.J., and fell into the vat. He was struck in the head by the large paddle used to mix the chocolate.

OSHA said that the facility where Smith died did not have a first-aid program in place and had improper safety precautions.

A city judge had already levied a fine for operating a business without a license.
